FP Associates, P.C. is an independent medical office in Bellevue, Nebraska, serving Sarpy County, Omaha metropolitan areas and surrounding communities in both Nebraska and Iowa.
Dr. James Cervantes and Dr. Brock LaSure are board certified family physicians and are medical staff members at the Nebraska Medicine Bellevue Hospital, Nebraska Medical Center, Midlands and Bergan Mercy Hospitals, and other area hospitals.
Drs. Cervantes and LaSure provide quality and compassionate medical care throughout all stages of life. We look forward to meeting you.

Family Physicians & Physician Assistant
James A. Cervantes, MD
Dr. Cervantes is a Board Certified Family Medicine physician who provides medical evaluation and treatments for patients of all ages. His areas of special interest include chronic disease management, preventative healthcare exams for both adults and children, men's health and sports medicine, and minor surgical procedures.
Dr. Cervantes has been providing medical care to families in Sarpy County since 1995 and has been practicing in Bellevue since 1997. He has patients in the surrounding communities and Omaha metropolitan area. He enjoys science and technology, sports, being outdoors, reading, and spending time with his family.
Brock R. LaSure, MD
Dr. LaSure has been practicing in Bellevue since 2006. He sees patients of all ages and enjoys managing all types of acute and chronic health conditions. He has particular interests in preventative care, dermatology, pediatrics, and orthopedics. His hobbies include travel, landscaping, movies, cooking, Nebraska football, and spending time with his family.
Carrie A Juracek, PA-C
Carrie is a highly qualified physician assistant and is available to treat your urgent medical needs. She sees patients of all ages for acute and chronic illnesses and injuries. She performs sports physicals, and annual physical exams for both men and women.
Carrie worked in rural South Dakota for two years before joining Dr. Cervantes in Bellevue in 2003. She enjoys spending time with her family.
